Online Hall of Fame Nomination Form

I/We would like to nominate to the Minnesota Blues Hall of Fame *

~ Candidate must have Blues as the primary format or style of music ~ Candidate should have at least a 20-year history of participation in Minnesota blues music ~ Candidate must have made a historical contribution and/or had an impact and overall influence on blues in Minnesota

Award Category

Performer: Celebrates the contribution to the blues of an individual artist or group.
Sideman: Awarded to a musician, primarily performing in the Blues genre other than as the leader or featured artist of the act(s) or group
Blues Recording: Awarded to a compilation of songs published and distributed as an album, CD, record, or other accepted formats
Blues Song: Awarded to the performer(s) and/or songwriter(s) of a song that was published and distributed on an acceptable format
Legends Award: Awarded posthumously to an artist who made a significant contribution to the blues genre in Minnesota.
Legacy Award: Celebrates the lifetime contribution to the blues of an artist who made a significant impact on the Blues genre in Minnesota during their lifetime.
Keeping the Blues Alive (performer) Keeping the Blues Alive Award: Recognizes the outstanding contributions that an individual or organization makes to the blues genre in Minnesota.
Supportive of the Blues (Non-Performer): Includes: agents, broadcasters (radio or television), historians, media outlets, music stores, promoters, radio stations, sponsors, teacher/educator, music venues, and other industry professionals
Blues Art, Literature and Media: Celebrates achievement of outstanding work of a person in the industry
